DFGT to PS4, Less than a minute of gameplay, infinite menu

Issue: Initially the software works just fine, but after a few seconds in game the inputs "lock" on. In other words, it's like at some random moment a few seconds into a race an event occurs and whatever input state my controller was in (throttle/brake input, steering angle, etc) is what the game constantly receives until I unplug the cable.
Unplugging both ends and plugging it back in resolves the issue for a few more seconds. If I leave the game at the menu it'll last indefinitely (at least a couple of minutes tested).
Not sure what to try. I've reseated the cables multiple times to make sure they were secure and have used multiple USB cables for the adaptors and controllers.
